Please fill out the New Patient Form at the link below. This will transfer your information into our system, and help make your first visit with us run more smoothly. Please make sure to complete the entire form, and click the “Submit Patient Information” button when you are done.
Referrals
For Wellness, Massage Therapy sessions* can be scheduled directly through our office, without the need for a referral from your doctor. These sessions will be on a self-pay basis. Commercial insurance and Medicare do not apply.
A referral is required from your physician for Physical Therapy sessions* regarding injury or chronic conditions. We will provide you with a superbill that you can submit to your commercial insurance as out-of-network. We do not accept Medicare patients.
*Both Massage and Physical Therapy sessions will incorporate Myofascial Release Techniques.
